The “Fukuro Lady” and the same monster from the music video is in SH3 elevator cutscene the very same from the intro. Heather is looking at them with disgust shocked etc. Later she is in the hospital on few places and pretty much at random places throughout the game especially near the end in nightmare worlds. If you look out of windows or fences at certain places.
