Rose runs into these guys during her first Otherworld transition in the dark alley. These creature were based on the Grey Child and Mumblers from Silent Hill

An armless, stumbling monster that spits eroding acid. Rose and Cybil run into these creatures on the outskirts of town. These creatures were based on the Lying Figures from Silent Hill 2

A humanoid man encased in barbed wire that moves slowly by dragging itself along the floor with its hands. It is able to deteriorate walls by touch and can call forth a mass of creeper. This creature was made especially for the film and represents the school janitor, Colin, who is implied to have molested a young Alessa. Rose runs into this creature in a Midwich Elementary bathroom.

Small beetle-like monsters that come in huge swarms and can eat a man within seconds once overpowered by their numbers. Rose runs into these guys in the Otherworld Midwich Elementary. These creatures were based on the Creepers from Silent Hill and Silent Hill 2

A large and muscular and humanoid monster with a black tetrahedral helmet and wields a very large sword. Kills in a variety of ways including slicing victims to pieces with his sword and ripping off their skin. Rose and Cybil run into this creature in the Midwich Elementary Otherworld and on the front steps of the church. This creature is based on the Red Pyramid Thing from Silent Hill 2

These humanoid creatures in short nurse uniforms and high heels attack fiercely with scalpels and are attracted to light. Rose runs into a large group of them in the Brookhaven Hospital basement. These creature wesre based on the Bubblehead Nurses from Silent Hill 2

Technically Darkside of Alessa is considered to be more then just a mere creature but I’ve put her in this section since she is the embodiment of the evil in Silent Hill. She doesn’t attack people directly instead she has others do her bidding. Rose sees her all over town during the movie and mistakes her for her lost daughter Sharon.

Technically the Scarred Older Alessa is a character in a the film but considering she acts like the “final boss” of the movie I thought she fit better within this section. Alessa is confined to a hospital bed but uses tendrils of barbwire to tear apart her enemies. Rose sees her during the take over of the church at the end of the film. Since Alessa was still confined to her bed there is a similarity to the design of the Mary Monster from Silent Hill 2
